This form IS for:
Those who would like copies of sacramental records from the Basilica's archives. These include weddings, baptisms, confirmations, first communions, and funerals.

This form IS NOT for:
Potential Godparents looking for a copy of their baptismal records. Instead, you need a letter of approval from your Pastor. If the Basilica is your home parish, please contact Bob Reier at 410-727-3565 x51 or reier@baltimorebasilica.org for this assistance. If you are not a Basilica parishioner, you will need to obtain permission from the Pastor of your current parish as well as obtain copies of your baptismal records from your parish of birth.

The fee for records research is $15, which can be made by check only, to the "Baltimore Basilica" and mailed to: Baltimore Basilica, Attn: Records Research, 408 North Charles Street, Baltimore MD 21201. Please do not call the parish office directly regarding requests for genealogy or records research.
